Billing & Rates

Travel Fee:

Dementia Dimensions is located southwest of Madison, WI. A travel fee will be applied if travel time exceeds 25 minutes from our location, billed in 15-minute increments.

Cancellation Policy:

To avoid a cancellation fee, please provide a cancellation notice via email or phone 48 hours prior to your appointment. If a non-emergency and no-show cancellation is made less than 48 hours of the scheduled appointment, a refund will not be provided. Payment is due at time of scheduled appointment.

Financial Policy:

Payment for services is due at initial appointment. Additional payment required if session exceeds the initial scheduled time.

Pricing:

Initial Consultation:
This will guide your personalized action plan. Follow-up notes, appropriate resources, education, and any other pertinent information will be provided after the consultation.

Cognitive Screening:
Screening for an individual concerned with cognitive challenges. A screen may indicate areas of cognitive deficit as well as how these challenges may affect their daily life.

Follow-Up Sessions/Additional Fees:

60 minutes: $100
45 minutes: $75
30 minutes: $50
15 minutes: $25

Frequently Asked Questions

  • No, all services are private pay only and not covered by insurance. Although I have a background in Speech-Language Pathology, my role is to provide dementia coaching and wellness services.

  • No, you do not need a referral for my services.

  • I may work with adults managing care for their loved one, an individual living with dementia/cognitive challenges, and healthcare/business professionals.

  • If service area is more than 25 minutes from Hannah's home base, a service fee will be added.

  • No. Hannah will be providing consulting, coaching, and education services specifically related to her clients’ needs. Hannah will refer to outside therapy sources if needed, such as physical, occupational, and speech therapy.

  • Please fill out the information on the "contact us" form, and I will be in contact with you as soon as possible.

  • I will initially discuss the reason you reached out for my services through email or a 15-minute phone call. If interested, you will then schedule an initial consultation (approximately 60-90 minutes) to discuss a plan of action. I will continue to help you as needed virtually, by phone, or in person.

  • This will be discussed during the initial call, as I aim to design a plan that will directly benefit and support you and others involved.

  • Yes, I can provide cognitive screenings; however, I do not diagnose cognitive disorders. I will assist you in locating the appropriate healthcare professionals to meet your needs.
